Skeletal anchoring

Skeletal anchoring

Orthodontist Dr Mehdi Elahi

The combination of skeletal anchorage and multi-bracket systems is ideally suited for difficult tooth corrections, especially in the posterior region. With conventional methods, the entire dental arch must be fitted with brackets for anchoring to correct posterior teeth.

With the help of skeletal anchorage, this is no longer necessary. Brackets are only bonded to the teeth to be moved. We use orthodontic anchoring implants, pins (mini screws) and Ortho-Bone-Anchor elements (OBA) for skeletal anchoring.

Example A

In this 17-year-old patient, the unfavourable occlusion resulting from the mandibular retrusion (Class II dentition) would have been treated classically by removing two permanent teeth in the upper jaw.

In his case, however, the entire row of maxillary teeth was moved back and an optimal occlusion was achieved without having to extract two healthy teeth. This treatment concept was made possible by the use of an anchoring implant inserted in the palate. The discrepancy in tooth size between the upper jaw and the lower jaw was equalised by building up the lateral incisor.

Start of treatment

After 2.5 years

Example B

36-year-old patient with palatal position of molars 17 + 18, residual gap of the extracted first molar (16) and crossbite of the molars in the maxilla on the right.

With the help of an OBA anchorage, molars were moved outwards and forwards, the crossbite was eliminated and the existing gap closed

Start of treatment
Palatal position of 17 + 18 with residual gap of 16 and crossbite of the molars on the right.

After 2 years
Removal of the crossbite and the gap in the maxilla on the right after 2 years.

Example C

In a 17-year-old patient, the first molar in the upper left jaw had to be extracted. The wisdom tooth was in place but had not yet erupted.

By moving the second molar forwards, the gap was closed and the wisdom tooth, which would otherwise have been extracted, was able to break through in place of the second molar and take over its function.

The technique used here is a combination of skeletal anchorage and segmental technique.
